We will also ask you for information about how the diagnosis was made -- for example, biopsy, x-ray, etc. -- the date of diagnosis, and the primary site of the child's disease.

It's important for us to know what treatment the child may have received for his or her current illness -- for example, chemotherapy, surgery, radiation.

Name and contact information for the child's physician and/or medical care facility, and the name and contact information for the physician and/or medical care facility that has treated the child for his/her current illness -- if different.

Child's name, date of birth, Social Security number, parents' names and contact information.

Child's Primary Medical Insurance Carrier
Memorial Sloan-Kettering Cancer Center works closely with insurance carriers if authorizations or out-of-network approvals are required. Our Patient Financial Services staff are available to respond to questions and assist with the authorization processes.
